OUR MEDICARE/MEDICAID LONG-TERM CARE PROGRAMS SERVE THE NEEDS of OLDER ADULTS and ADULTS WITH DISABILITIES. WE COORDINATE and DELIVER A FULL RANGE of SUPPORTIVE SERVICES THAT HELP THEM LIVE SAFELY, CONFIDENTLY and WITH DIGNITY.
